We will require your insurance information to bill for the service or you may pay by credit card without filing through insurance. If your insurance does not cover or denied claim then you will be billed $100 for the visit
Primary Care

We will obtain comprehensive history and we may ask you to provide us previous medical records.

We will take your comprehensive health history via our patient portal of electronic medical records to gain insight for your health.

We will develop personalized preventive and screening plan of care for your health.

We will ask for medical records and other information from your primary care provider or other providers you may have seen.

We will require your insurance information to bill for the service or you may pay by credit card without filing through insurance. If your insurance does not cover or denied claim then you will be billed $100 for the visit if an established patient and $200 if a new patient.
Post Acute Care

You can request an appointment while you are in the hospital or after you get discharged.

As your primary care Physician or Physician representative, we can co-ordinate care with your inpatient doctors or consultants.

We will request your hospital records for review.

We will organize your complex health data into simple format for you to understand and also explain to you and family members as needed.

If you do not have insurance or insurance does not cover the visit, you will be billed $100 for the visit if an established patient and $200 if a new patient.

We can also provide you 2nd opinion on several health conditions as long as those fall into our area of expertise.
